AGED CARE - CARE TEAM MANAGERS

Sydney and North Coast NSW

AHPRA Registered Nurses

  • Wallace Street Recruitment are looking for an enthusiastic and experienced Aged Care Registered Nurses for our client, a well-respected Aged Care provider that provides services throughout NSW
  • We have roles available in Sydney also on the North Coast of NSW
  • Reporting directly to and in conjunction with the Facility Manager, the role as the Clinical Team Manager is to lead and manage a care team to deliver safe and effective clinical and personal care to residents that meets their individual needs and preferences and optimises their health and wellbeing
  • You will be responsible for the safe and effective delivery of clinical care including, but not limited to, clinical leadership and management of resident care, ensuring your own and team clinical expertise, clinical education of care service staff, medication management, case management, GP liaison, and discharge planning
  • You will possess and maintain expert knowledge and understanding of the responsibilities under the Aged Care Act 1997 and the Aged Care Quality Standards 2019, including residents’ rights and accreditation requirements, and deliver care services accordingly to ensure accreditation at all times
  • You will possess a very good understanding of ACFI requirements, to ensure clinical documentation supports the ACFI funding is completed to meet ACFI targets and, assist the Facility Manager throughout accreditation and the auditing process.
 

Essential Criteria:

  • AHPRA Nurses’ Registration – unrestricted
  • A minimum of 4 years recent experience within the Aged Care sector
  • Sound knowledge of ACFI, Care Planning, Aged Cares Standards and auditing
  • Experienced in management and leadership within the aged care setting
  • An understanding of the impact of complex care needs and a demonstrated ability to apply a high level of clinical knowledge (including palliative care and dementia care)
  • A willingness to work after hours when needed and occasional on-call
  • Excellent leadership, interpersonal and communication skills
  • Criminal History Record Check
